LLD Construction, LLC otherwise is name is unknown, and address is 7658 Catawaba Lane, Apt 10, Florence, KY 41042-1878, otherwise whose address is unknown, will take notice that on April 16, 2025, the undersigned, Hector Lopez, filed a complaint in the Hamilton County Court of Common Pleas, 1000 Main Street, of Hamilton County, Ohio, 45202, Case Number A-2501912, alleging that on or around 8/28/2019, Plaintiff was employed by the Employer, LLD Construction, LLC and while performance of duties incidental to and arising out of the course of his employment, injured himself; that a direct and proximate result of said injury, it was necessary for him to seek and obtain medical and hospital care and was disabled therefrom; and that following the aforementioned accidental happening, and within the time provided by law, he did file his application for payment of compensation and medical expenses, which application was assigned Claim No. 20-161869.
Plaintiff prays that a jury be impaneled and sworn to determine the rights of the Plaintiff to receive compensation and medical expenses; that he be awarded a finding that he is entitled to participate in the State or Self Insured Insurance Fund, and that such findings be certified through the Industrial Commission of Ohio and the Defendant, Administrator of the Bureau of Workers’ Compensation, as provided for by law together with attorney fees and judgement for the costs of this action.
The defendant named above is required to answer on or before the 14th day of November 2025.
